Bargaining Unit: EARate of Pay: $27.76/hour + DOESummaryThe Community Health Program Coordinator participates in the development, implementation and evaluation of health and wellness programming to improve the wellbeing of our community members and address the needs identified in the triennial Community Health Needs Assessment. Provides administrative support related to accounting, coordination of meetings and community outreach.Essential Duties and ResponsibilitiesParticipates as a key member of the Community Health team in the planning, development, coordination and promotion of community health and wellness activities.Responsible for the planning, coordination, implementation, and evaluation of health interventions including, but not limited to: the Harvest of the Month Program, B-FIT program, Affordable Labs, and employee produce program.Develops curriculum,Produces internal and external communicationOrders and distributes suppliesCollects and reports on health intervention evaluationsTracks and reports on dataPlans, assigns work, and coordinates logistics to implement successful community programming and events.Collaborates with team members to facilitate Truckee Thursdays Booth, School Wellness Days, nutrition events, school dental screenings, and other community wellness activities and events.Adheres to program budgets.Completes monthly data analysis and reporting on department events, classes, and outreach.Identifies areas for program improvement and participates in Quality Improvement initiatives.Provides support with billing adjustments and dispersing results for the Community Health Labs.Verifies appropriate charges, complies results, and mails to participants.Maintains knowledge of upcoming events, services and information for all department programs and services.Assists in the development of creative and engaging marketing materials, communications, and patient/provider education to promote programs, community challenges and events to maximize engagement and represent the Health System's commitment to community health.Researches, assembles and prepares presentations, reports, agendas, minutes and materials for internal and partner/community meetings and coalitions as assigned.Orders supplies and maintains appropriate inventory levels.Demonstrates System Values in performance and behavior.Complies with System policies and procedures.QualificationsTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
