Rehousing Programs

Five Keys has case management contracts with the San Francisco Department of Homelessness and Supportive Housing (HSH) for a number of re-housing programs designed to place people who are unhoused or are guests in temporary shelters into permanent housing on the private market with temporary subsidies or permanent vouchers. These programs include Rising Up for transitional aged youth, Adult Rapid Rehousing, Flexible Pool Subsidy Program, and Emergency Housing Vouchers for returning citizens. Five Keys is contracted to provide individualized care management for participants who are placed into these programs through the coordinated entry system. These programs are complex partnerships between various organizations in San Francisco– including HSH, the Office of Economic and Workforce Development, the SF Department of Public Health, and multiple community-based agencies that provide housing location and rental subsidy services.

JOB SUMMARY

The Assistant Director of TAY Rapid Rehousing reports to the Director of Rehousing Programs and is accountable for the comprehensive management of multiple re-housing contracts. This position provides daily supervision for a team of supportive services case managers and represents Five Keys regularly at partnership and stakeholder meetings. Key responsibilities also include administrative support related to program policies, partnerships, and outcome monitoring. The ideal candidate will be an excellent leader and will have experience in managing different programs and responsibilities. They will support staff members to strive to raise the standard of professionalism and ensure the highest standards and opportunities are offered to our program participants, our partners, and fellow staff.

JOB DUTIES AND RESPONSIBILITIES

Programmatic

  • Collaborates with the Director of Rehousing and the other Five Keys Leaders to explore strategic opportunities and innovative approaches that enhance or expand services for housing guests and program participants
  • Collaborates with program partners, including local, county, and federal housing agencies, to ensure quality service delivery, while fostering innovative thinking, cooperation, and service integration among staff, partners, programs, and other Five Keys divisions
  • Develops and manages the care coordination policies and procedures to ensure that Five Keys meets contractual obligations and delivers high quality services to program participants
  • Recommends new approaches, policies, and procedures to effect continual improvement in department efficiency and service performance
  • Supports staff by ensuring a work environment marked by positive communication, participation, and opportunity for professional development
  • Works closely with leadership and employees to improve work relationships, build morale, and increase productivity and retention
  • With the support of the Director of Rehousing Programs and government housing agencies, ensures effective service delivery by overseeing development and management of quality assurance and program evaluation
  • Attend assigned partner and stakeholder meetings and represents Five Keys with professionalism
  • Represents Five Keys in partnerships, networks, and coalitions to promote positive and collaborative working relationships that support rehousing programs
  • Assist in the development of a community resource list
  • Completes other duties as assigned

Administrative

  • Ensures staff are properly trained on timely, accurate data collection policies, procedures and strategies
  • Provides oversight of placement and outcome data through routine monitoring and staff coaching
  • Effectively engages and motivates external partners and staff to enhance the quality and efficiency of rehousing care management services.
  • Ensures the program is prepared for external program monitoring, including audits by funding and other agencies
  • Ensures safety procedures are followed by employees, subcontractors, and care management staff
  • Takes ownership of the program's administrative processes, including updating procedures, instituting new policies, and providing staff training and coaching as needed

Fiscal and Regulatory

  • Maintains productive working relationships with key private and public funders
  • Maintaining documentation and financial records in compliance with audit standards and responding to audit findings
  • Ensuring timely payment to landlords, vendors, and service providers
  • Monitoring spending against budgets and ensuring timely and accurate recordkeeping (e.g., rent payments, utility assistance, deposits)
  • Ensures program is fully compliant with contractual obligations, provides daily oversight, and adjusts policies and procedures as needed
  • Works with the Director of Rehousing Programs to track outcome objectives
  • Prepares reports on care coordination services, gaps, and areas for improvement to Five Keys leadership, funding agencies, and other partners
  • Ensures all care management staff remain compliant with training requirements, including harm reduction, the administration of NARCAN, and de-escalation strategies
  • Other duties as assigned

KNOWLEDGE AND SKILLS

  • Experience working with individuals experiencing homelessness, with a strong understanding of their unique needs and the supportive services required for successful program engagement
  • Experience working in harm reduction environments and supportive housing placements
  • Advanced problem-solving and collaboration skills, ability to think critically to resolve concerns tactfully and efficiently and achieve shared goals
  • Excellent written and verbal communication skills with the ability to effectively collaborate
  • Ability to follow and provide verbal instructions in emergency situations
  • Strong interpersonal and communication skills, including active listening and high emotional intelligence, and a collaborative mindset.
  • Demonstrate a strong willingness to learn and the flexibility to adapt effectively in a dynamic, constantly evolving environment
  • Advanced proficiency in Zoom, Microsoft Office, Google Workspace, and using databases to track program outcomes
  • Able to maintain strict confidentiality and take all precautions when handling sensitive information
  • Ability to maintain professional, respectful, honest interactions with participants, staff and outside agencies
  • Ability to work effectively as part of a multidisciplinary team and demonstrate cultural sensitivity when engaging with a diverse population of participants and staff
  • Possess excellent charting, record-keeping skills and the ability to train and coach staff
  • Must have a solid understanding of and sensitivity to the challenges faced by individuals with substance use and/or mental health conditions, such as PTSD, psychosis, bipolar disorder, and borderline personality traits
  • Strong knowledge of mental health resources available to low-income adults
  • Skilled in crisis intervention techniques and capable of applying them effectively
  • Strong knowledge of social services and area resources

EDUCATION AND WORK EXPERIENCE

  • Bachelor's Degree with a minimum of 1 year of paid experience serving homeless populations, or
  • Associates Degree with a minimum of 3 years of paid experience serving homeless population, or
  • 5 years of paid experience serving homeless populations
  • Supervisory experience is highly desired

PHYSICAL REQUIREMENTS

  • Must wear provided PPE (Personal Protective Equipment) and adhere to all safety guidelines and requirements, when needed
  • Moderate to extensive degree of physical and emotional stamina
  • Frequent talking and hearing conversations
  • Long periods of walking, sitting at a desk and/or computer station
  • Lifting items that weigh up to 25 lbs; reaching, bending, carrying and stooping within the daily routine of tasks and activities

ADDITIONAL REQUIREMENTS

This position is contingent upon:

  • Submitting proof of clear TB results. Results dated within 60 days prior to hire date are acceptable

WORK ENVIRONMENT

  • Working in homeless services locations with guests that may have had exposure to COVID-19
  • Must be willing and able to travel throughout the area to community sites
  • Work environment varies from site to site
  • Some personal safety risk working in underserved communities
  • Working in a very high risk environment where it is possible to be exposed to COVID-19
  • Moderate to high noise level
