Job Details

The Diabetes Educator is responsible for coordinating and conducting individual and group patient assessment and education for both diabetes and other/like chronic health conditions. Works in collaboration with physician(s) in the clinic setting to evaluate patient needs and individualize patient education. Patient education may include but is not limited to diabetes self-management (blood glucose monitoring/interpretation, carbohydrate counting, medication action/ usage and acute and chronic complications) diabetes and pregnancy, support groups, data analysis and management of patient blood glucose levels with physician orders. Nature and Scope Demonstrates the knowledge and skills necessary to provide care, based upon physical, motor/sensory, psychosocial, and safety appropriate to the age of the patient served.
